    On the guitar, there are a few possibilities....

    Dm#7#11/A | x-0-0-6-6-4 | or | x-5-7-6-6-4 | - G# on top played with the side of the index finger.

    or: | x-0-3-6-3-4 | - et. al.

    Thanks for your reply. As I mentioned in my OP I'm bedridden so I can't play an instrument.

    My condolences, get well soon, I think GP8 can produce an audio file, I'll try that.

    Thanks Mick, you're very kind. FYI I'm five days post-op from a multi-level spine surgery plus removing some hardware from the previous one. Prognosis is quite good. Just gotta be careful. Best to you.

    Thanks very much, Mick. Could well be me, but the sound I'm hearing is the "spy chord" with a raised 11th. I dunno, maybe they should have operated on my ears. I was imagining something closer to the ending chord (4:51) here:

    Did I play the right chord? Are you asking for a minor chord with a major 7th & #11th or something else? - Dm#7#11 = D-F-A-C#-G#.

    Chord in the video you posted sounded like a min9#7, 9th on top, no #11th, for example: G-F#-Bb-D-A - a great ending chord.

    Actually, you could add the #11th (C#) on top, above the 9th, don't think that can be played on the guitar though....

    Yes, it's possible, e.g., Em9#7#11 | 0-2-1-0-2-2 |

    Well, as you can tell, I'm not quite sure what I'm asking for. I'd reckon you're probably correct on the base chord. And, yeah, it was more about stretching my ears than my fingers. Must be the meds.

    Fingering is only a problem on the guitar but this voicing I posted has all the notes [E-B-D#-G-C#-F#]:

    Em9#7#11 | 0-2-1-0-2-2 |

    It's a different sound if you omit the 9th as I did in my first 4 voicings.

    OP here. I found a build-a-chord app online. The sound I was hearing is a minor/major7add9. No #11th. Appreciate all who responded.

    That's a chord I use as a 'final strum'. It's very nice.

    So, to put this in a key, Dminmaj7#11 would be D F A G# C#. If one note had to go, I'd guess it would be the A natural.

    So, what about D F G# C#? Might be worth noting that C# major triad is C# F G#. If you put the A back in, it looks like Amaj7#5.

    And, if the bassist plays a D (or you do), you get Amaj7#5/D. That's D A C# F G#. Or, respelling, D F A C# G#, which looks like Dminmaj7#11, proving nothing. I'd leave the D for the bassist and maybe try x8769x.

    To hear the entire chord try moving it up a step to E G B D# A# and play 0 10 9 8 11 x.

    Without the lowest note it sounds like a good voicing for the opening of Nica's Dream. And, then I realized it is the exact chord I usually play there. xx5446.

    Maybe, maybe not, you will probably want the contrast of the A vs. G#, as in a Maj7#11 chord. You could omit the root of the chord (D).

    Yes, we can usually do just fine with abbreviated versions of such big piano chords.

    Speaking of Nica, the New Real Book v2 has Dbm9(Maj7)b5 in bar 7 of the B section, the 9th (Eb) is in the melody. Real Book 1 has DbM7.

    So: Dm9#7| x-4-3-5-4-x | vs Db^7| x-4-2-5-4-x |.
