I've seen more than a few inaccurate transcriptions online, but not often in books, nobody wants to see their errors in print.

Getting Transcriptions Wrong

Actually this was the rehearsal at the venue before the gig. On the gig I had long slacks, button cotton shirt but no shoes! I never wear shoes.
