The head means the melody of the tune (in theory, something easy enough to keep in your head instead of needing the music in front of you, or so I'm told). A rhythm changes head is any melody that goes with a I vi ii V progression. Off the top of my head, I Got Rhythm, Oleo, Dexterity, and Anthropology come to mind.
