Dude you need a nice trainer.. he can get you all, coordination, speed, flow and can even develop your personality.

We can't really help you if you don't give us what to work on. What exactly do you need help on? How good are you at these things, or what are your strong points?

1-4 practice slowly, practice doesn't make perfect, perfect practice does.

The first 3 can easily be accomplished by working with a method designed to target strength, dexterity and coordination. Jody Fisher's 30 Day Guitar Workout is something I have used to good result for years myself and with students.

In terms of your own voice or style, that is a bit trickier, and takes a while. Often what happens, is people emulate one or more of their favorites, and wind up melding bits from more than one source. Our own personalities, and technical idiosyncracies tend to spice up the mix also. Spend some time learning the material of one of you favorite players.

An example might be Mimi Fox. She started out as a star student of Joe Pass, but evolved into her own thing after a while. Good luck

Jamie Andreas, 'The Principles of Correct Practice for Guitar'. While it is not a jazz guitar method I have often wondered why this amazing book doesn't get mentioned more for it's timeless advice on how to solve issues relating to points 1-3. Point 4 is too vague. Point 5 can't be taught. If you put in the consistent effort necessary then your style will emerge in due course.
