I don't want to dominate the responses.
So just one for starters. The first place I noticed the repeating roots was measure three, beats 3 and 4. Both have the bass note D. IMO, it is more common to walk the D to C, and then on the next beat to Bb, for Bbmaj7 (to harmonize the high D-note). (Some call this a "cliche" line). It's a small counterpoint of one line going down while the other goes up. That thump, thump consecutive D's in the bass is not "jazzy." Also, I think you are harmonizing the high D-note with G min7 not Bbmaj7, which doesn't matter. (I'll get back to that).
I never tried to use the tablature function on this site, so here is the simplest possible way I can think of, two notes at a time. Starting in the same position V, where you were:
On beat 3, play D on the G string (with 3rd inger), to harmonize the F,G melody notes.
On beat 4, drop the D to C (1st finger, mini-barre)), and harmonize the A,C melody notes.
On beat 1 (meas 5) slide up to pos VIII and play the Bb on the D string (with 1st finger) to harmonize the high D.
You can either fill out the middle voices between Bb and D to make Bbmaj7 or Bb6 if you prefer (which is the same as the Gmin7 you used on the Tube, in a different inversion).
